Participating in this survey this you a chance to leave valuable feedback that the company uses to help make better products and give better customer service. You will also receive a 15 percent discount coupon for your next purchase. You will also be entered into a drawing for a $500 JCPenney gift card. You are not required to enter the drawing in order to fill out the survey and leave your feedback.

- Click either English or Spanish, whichever is more comfortable.
- Enter the survey invitation code in the box provided. For help, see the diagram beside the boxes. Click continue.
- You will be asked a series of questions about your most recent visit to the store. Please answer as honestly and as thoroughly as possible. Before moving to the next series of questions, you must answer all questions on the current page.
- At the end of the survey, you have a chance to enter the gift card drawing. Select the best answer and move to the next page.
