ELIGIBILITY: Survey is offered to all legal residents of the 50 United States, D.C., who has
the age of majority in his or her state of residence at the time of entry and who are not employees
of Cheddars Casual Café Inc., (“Sponsor”), its parent companies, subsidiaries, affiliates,
advertising, research and promotion agencies, and others involved in this drawing. Immediate
families of persons above (spouses, children, and household members) are not eligible. Survey is

2. PARTICIPATION PERIOD: Invitation begins at 12:01:01 a.m. on November 1, 2011, and
ends at 11:59:59 p.m. on December 31, 2013. A Quarter in the Participation Period will contain
three months. The completion of each Quarter will be on the following four dates: (January
31, 2013, April 30, 2013, July 31, 2013 and October 31, 2013) (the "Promotion Period"). A
Month in the Participation Period (“Month”) begins at 12:00 a.m. (midnight) on the first calendar
day of the Month and ends at 11:59:59 p.m. on the last calendar day of the same month.

the requested information from your receipt to be automatically entered. Limit of one (1) entry
per household per month, e-mail address and/or mailing address accepted for this
Sweepstakes. In the event of a dispute over the identity of an online entrant, entry will be
deemed submitted by the "Authorized Account Holder" of the e-mail address submitted at time
of entry. Authorized Account Holder means the natural person who is assigned to an e-mail
address by an Internet access provider, online service provider, or other organization that is
responsible for assigning e-mail addresses for the domain associated with the submitted e-mail
address. Entry materials/data that have been tampered with or altered, or mass entries or entries
generated by a script, macro or use of automated devices, are void.
B). To Enter Without Purchase: You may enter by printing your name, address, day and night
telephone numbers and age on a 3x 5card and mailing it in a standard business-size #10 envelope
to: Cheddars Casual Café Inc., “Customer Satisfaction Survey” 90 Rockwood Place, Rochester,
NY 14610. All mail-in entries must be postmarked by the quarter end date and received
within five

ReplyDeleteFood was brought cold. Baked potatoes would not melt butter. Floor was slippery waitress was falling. Floor was not c!earned after fall no concerns for safety of people. Do not foresee Cheddars in Winston Salem open long.
