The responses provided in the Adidas Feedback questionnaire used by ForeSee and Adidas to measure customer satisfaction, and will otherwise be kept confidential. The customer only has 7 days from the date of purchase to take the survey or it will become void.
Items needed to take the Adidas feedback survey?
- Date of purchase
- The store and transaction number
- Age
- Internet access
At the end of the survey customers will be entered into a sweepstakes for his or her choice of one (1) pair of shoes from the store brand (Adidas, Reebok, or Rockport). The customer can pick a shoe worth up to $120 but depends on the model/style/availability of shoes selected. Please note ff the winner selects a $80 shoe they will not be reimbursed for the remaining $40. ALSO the customer must elect to join the Adidas Feedback sweepstakes and by doing so will have to provide his or her complete first and last name (no initials), valid e-mail address, complete street mailing address (no P.O. Box), ZIP/Postal Code, telephone number and age.
Other Rules and Regulations
- The random prize draw will usually take place on the 9th, 10th or the 11th of the month
- Canadian winners will have to complete an annoying mathematical skill testing question before they are allowed to collect the prize
- The prize will be awarded as a voucher which is good for a purchase of up to $120 at Adidas
- Total amount to be given away during the course of the Adidas feedback sweepstakes will total $2,400
- Winners will only have three (3) calendar days to accept the prize
The name of winners can be requested by writing to Adidas Survey Sweepstakes, P.O. Box 251328, West Bloomfield, MI, USA 48325. Requests must be received by February 1, 2015.
